CCHN Services

The Community Clinics Health Network (CCHN) is a non-profit 501(c)(3) subsidiary of the CCC that was founded in 1993 to provide managed care support to San Diego's community health centers. 

Today, the mission of CCHN has broadened with activities to enhance quality of care, improve population health outcomes, and strengthen business efficiencies by offering specialized programs, services, and technology solutions to over 15 community clinics and health centers.  CCHN provides technical expertise in financial, quality, and operational management, and managed care support including contracting and utilization review for member clinics.  CCHN also provides quality improvement and disease management services and assistance to participating community health centers. 

It is the vision of CCHN to be recognized as a national leader for creating model programs, sharing expertise, and providing exceptional services in collaborative healthcare ventures that result in stronger community clinics and health centers, as well as healthier communities.
