Diabetes Patient Education Documents
Diabetes Patient Education Handouts were developed as a collaborative effort of the Council of Community Clinics and The Whittier Diabetes Institute with support from a HRSA HCAP grant. A series of 22 documents were translated into eight languages. English and Spanish versions can be downloaded from the links below. Arabic, Chinese, Laotian, Somalian, Tagalog and Vietnamese are accessible through The Whittier Diabetes Institute, Project Dulce.
