The CCC, CCHN and community health centers participate in a number of partnerships, collaboratives, and activities to provide quality health care to underserved populations and to improve population health outcomes. Staff works with other key healthcare stakeholders at the local, state, and federal levels in areas including advocacy, emergency preparedness, quality improvement, disease management, health education and case management. 

On the local level, we work closely with health improvement organizations such as Project Dulce to increase access to care for low-income patients with diabetes; the American Lung Association to provide enhanced treatment services and case management to children with asthma; Scripps Mercy hospital to increase access to specialty care, to name a few. 

On the statewide level, we actively collaborate with other clinic consortia such as the Community Clinic Association of Los Angeles County and the Coalition of Orange County Clinics.  CCHN and clinic staff works closely with the California Primary Care Association (CPCA) to improve health outcomes in a number of different areas including quality improvement, disease management, information technology, and strengthening partnerships. 

On the national level, CCHN participate in the National Association of Community Health Centers (NACHC) network to reach out to clinics throughout the country.
