Diabetes

The Council of Community Clinics is participating in a state-wide Quality Improvement project with the California Primary Care Association (CPCA) and 13 other consortia to use standardized data collection and reporting for diabetes.  Accelerating Quality Improvement through Collaboration (AQIC) is funded through a grant by the California Health Care Foundation. This project will advance the collection and use of clinical data measures for community clinic patients through electronic data reporting systems emphasizing the Chronic Care Model.
