CCHN has been awarded a grant from Kaiser Permanente (KP) to expand the ALL (Aspirin, Lisinopril, Lovastatin) program to community clinics and health centers throughout Southern California.
The ALL program has been successfully implemented with San Diego clinics and patients over the past five years. The program is aimed at improving medication compliance and health outcomes for patients with diabetes and/or cardiovascular risk factors. CCHN will expand the ALL program to interested clinics through Southern California with the goal of improving health outcomes of patients at risk for cardiovascular disease (CVD).
The ALL program was developed in the Kaiser Permanente Health system to target members at high risk for cardiovascular disease (CVD). The program has been modeled to provide an evidence based, cost effective method of CVD prevention in these high-risk individuals. By implementing this program, KP has significantly increased use of preventative medications for CVD, created a replicable model, and has demonstrated leadership in CVD population care prevention.
The proposed project will identify patients at high risk for CVD in community clinics and health centers, and apply the ALL intervention as an evidence-based, cost effective means of improving health outcomes. The goal of the project is to improve health outcomes of community health center patients age 55 years and older at risk for cardiovascular disease (CVD).
The community clinics and Kaiser Permanente have a long history of working together in partnership to improve the health of patients and communities. Together, the two groups share a common mission and a similar diverse patient population.
The ALL program is just one example of Kaiser Permanente's leadership and commitment to share knowledge, expertise and resources and to build the capacity of community clinics to improve the health of their patients. The ALL program has demonstrated improved provider adherence with an evidence-based clinical protocol and improved patient adherence with medication, resulting in improved patient health outcomes.
By taking a proven clinical protocol and successfully implementing it in a community clinic setting, Kaiser Permanente and the ALL program have created a win for everyone involved. Clinic providers learn from Kaiser Permanente's success and clinic patients experience improved quality of care and health outcomes.
CCHN continues to hear from our providers how much the ALL program has benefited their patients and from clinic patients how much the ALL program has benefited their lives. CCHN is looking forward to continuing to work with Kaiser and clinics throughout Southern California to expand the program to additional patients.
