About CCHN

With substantial support from the California Endowment, the Council of Community Clinics (CCC) formed a 501(c)3 subsidiary, the Community Clinics Health Network (CCHN), to assist CCC members with the transition of San Diego's Medi-Cal patients to managed care.  Since its inception in 1993, the CCHN has evolved into a central support organization for participating community health centers.  The CCHN has regularly scheduled meetings with the clinic Executive Directors, Chief Financial Officers, Medical Directors, Human Resource Directors, Chief Information Officers and other IT Staff to share best practices, benchmark data, identify and develop training opportunities, and seek solutions to common problems.

One example of how the CCHN works collaboratively is the Physician Council.  Comprised of  clinic medical directors, the Physician Council, supported by CCHN staff, has developed and implemented standardized quality improvement and disease management programs for clinics in a number of areas including: diabetes, asthma, and hypertension.  Each year, the Physician Council and CCHN conduct a day-long retreat in determine the current direction of quality and disease management initiatives, and set priorities for the upcoming year.
