Connecting Quality and Technology for Improved Health Care
CCHN offers programs and services in quality improvement, operations support, managed care contracting, disease management, and information technology services to participating community health centers. It is the vision of CCHN to be recognized as a national leader for creating model programs, sharing expertise, and providing exceptional services in collaborative healthcare ventures that result in stronger community clinics and health centers, as well as healthier communities.
In collaboration with Kaiser Permanente, the CCHN is implementing the
Aspirin, Lisinopril, Lovastatin (ALL) Intervention in community health centers in San Diego and Imperial Counties with the goal of improving health outcomes of patients with diabetes at risk for cardiovascular disease.

Our latest initiative, Electronic Health Record (EHR) Planning, has been awarded California Networks for EHR Adoption planning grants for both the San Diego and Imperial County collaborative as well as the Los Angeles collaborative.
The Community Clinics Health Network (CCHN) offers a wide array of menu-driven services designed to assist community clinics with their business, clinical and information technology (IT) needs.
